Basic Care, Dementia Care or Long Term Care Services

If you are interested in submitting an application to Basic Care, Dementia Care or Long Term Care you must complete the following:

Step 1:  Admission requires an application. Applications can be obtained through the Social Service Admissions Coordinator. Please call (701) 572-6766 and ask for Connie Jupka or contact her via e-mail at admissions@bethellutheranhome.com .

Step 2:  Upon completion of the application, the individual will be placed on our waiting list. As a bed becomes available the family will be contacted and a clinical evaluation will be completed.

Step 3:  At the time of admission a physician order, history/physical and other medical information as requested are required.

Adult Daycare/Respite Care

Adult Daycare is designed to serve persons who return to their own home or care setting each night. If you are interested in this service or know of someone who would benefit from daycare, please contact our Social Service Admission Coordinator at (701) 572-6766 or via e-mail at admissions@bethellutheranhome.com .

Respite Care is designed to provide short term care (defined as less than 14 days) when it is necessary to relieve family members or other persons caring for an individual at home. If you are interested in this service please contact our Social Service Admissions Coordinator at (701) 572-6766 or via e-mail at admissions@bethellutheranhome.com .

Independent Living Apartments

If you are interested in submitting an application to one of our three independent living apartments, please call Bethel Lutheran Foundation at (701) 572-0223 to set up an appointment for a tour of our facilities or stop at the Foundation Office located at 1705 3rd Ave West. Office hours are 8:00 a.m. - 5:00 p.m. Monday through Friday.  You may also contact our Foundation Office via e-mail at foundation@bethellutheranhome.com .
