Basic Care is delivered in two quality settings. The Leonard Nelson Basic Care Addition provides a quality living environment through the newly constructed (2002) addition that includes 13 private and 6 semi private rooms. Horizon Manor, located on the 4th floor of Horizon Tower includes 3 private and 3 semi private rooms. Bethel Basic Care Services provides a quality alternative for individuals unable to function in the Independent Living environment yet not in need of skilled long term care services.
North Dakota defines a Basic Care Facility as one that provides services to individuals who, because of impaired capacity for independent living, require health, social, or personal care services, but do not require regular twenty-four hour medical or nursing services and makes response staff available at all times to meet the 24 hour per day scheduled and unscheduled needs of the individual.
Elopement
Safety precautions will be in place to protect our residents and prevent elopement and/or wandering to or in inappropriate settings.
This facility will strive to prevent elopement, but cannot guarantee it will not occur.
Basic Care Rate Schedule
Residents eligible for the Basic Care Assistance program are charged a daily rate determined by the ND Department of Human Services through a cost reporting process. Base rates for semi private rooms are $79.37/day with additional charges for private and preferred location rooms.
