Bethel Lutheran Home provides care through its licensure of 159 skilled care beds. Services are provided through neighborhood settings including:

  Admissions

       

  Adult Day/Respite Care

       

Horizon Tower with 2nd, 3rd and 4th floor delivering care through 32 single and 9 double occupancy rooms;

  Basic Care

        Legacy Square with 28 single and 2 double occupancy rooms;

  Child Care

        Wheatland Square with 24 single and 4 double occupancy rooms;
  Dementia Care         Harmony Square with 21 single and 6 double occupancy rooms; and
  Eden Alternative         Pine View Court a 24 semiprivate bed dementia unit.
  Employment/Volunteer        

The neighborhoods deliver care through quality staff who are trained to meet the individual needs of our residents. Staff assigned to the individual neighborhoods include nursing, social services, recreational therapy, housekeeping and laundry. Additional support staff include rehabilitation therapy, nutritional services and environmental services. Each neighborhood has its own dining room and neighborhood care station with decor that is specific to each area.

Elopement

  Pastoral Care         Elopement occurs when a resident wanders away, walks away, runs away, or otherwise leaves a care giving facility or environment unsupervised, unnoticed, and/or prior to their scheduled discharge.

Safety precautions will be in place to protect our residents and prevent elopement and/or wandering to or in inappropriate settings.

This facility will strive to prevent elopement, but cannot guarantee it will not occur.

Skilled Nursing Care Rate Structure

          The rate(s) charged residents in skilled care is determined for each individual based on the intensity of cares and services provided to that resident. There are 34 rate classifications ranging from $124.74 to $289.88/day. The facilities rates are based on costs for the following services and supplies:

 

       
         
  • Direct Care1 - Nursing and Therapies
  • Other Direct Care - Food, Laundry, Social Service and Activities
  • Indirect Care - Dietary, Housekeeping, Chaplain, Plant, Medical Records and Administration
  • Property
 
 
  1The Direct Care component varies based on the resident's level of care. Residents are assessed quarterly and their rate classification remains in effect for 3 months.
